One-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * certified diabetes educators with registered nurses and registered dietitians on staff * visits with an endocrinologist (diabetes specialist) with a referral from your physician

Eligibility - Population(s) Served: Adults 18 years and older with pre-diabetes, type 1 diabetes and type 2 diabetes
Application: Self-referral or by referral of a physician or nurse practitioner
